I ACCEPT the one-time annual scholarship award of $6,000 and understand that I MUST reapply by February 1st each year.
I DECLINE the Hammons Scholarship.

All scholarship awards are made based on the information provided on your scholarship application. We are assuming that your county of residence and SAI score entered are correct and that you are in either your 1st or 2nd year of school. We reserve the right to retract a scholarship should we find that the information has changed. Based on these qualifications, please accept or decline.

I do understand that in order to maintain this scholarship, there are certain expectations:

1. I must provide at least 25% of my tuition costs or the Hammons scholarship will be reduced in proportion to other scholarships/grants received.

2. I understand that my university will return all or part of my Hammons scholarship money if needed to meet the 25% tuition requirement at any time.

3. I must maintain a cumulative GPA of at least 2.0 or better.

4. I will continue to be an active member of a local church and maintain a high moral character and strong work ethic.

5. I must reapply each year by February 1st for the following academic year.
